Set between the events of Saw I and Saw II, the narrative follows a desperate and sick John Kramer as he travels to Mexico, seeking a risky and experimental medical procedure as a last hope for a miracle cure for his cancer.
However, John soon discovers that the entire operation is nothing but a scam, exploiting the most vulnerable individuals desperate for a cure. This revelation ignites a newfound purpose within him, leading to the return of the notorious serial killer.
